2006 Convention Hotel Reservation Form/Delegate Cancellation Notice:

You can select the 2006 Hotel Reservation Form PDF file, complete, and print it.  You can also cancel by filling out the 2006 Convention Delegate Cancel Notice PDF file.  If you have a digital signature, you can also digitally sign either of these forms.  If you are interested in getting a digital signature follow this link: http://www.digsigtrust.com/home.html. This is the company Branch 142 is required to use by the Department of Labor to digitally sign our LM-2 report.  Be advised that there may be other companies that provide digital signatures.
